New Patients


Thank you for your interest in being a patient at our clinic. Becoming a patient is an easy three step process.

  1. We receive your referral and contact you (or patient calls us when they are notified that they are referred)
  2. Patient completes registration document and returns it to us
  3. We call patient when receiving the completed registration and schedule appointment with one of our providers

THE REFERRAL: Patients are typically referred to this clinic by their PCP or other specialist by sending us a referral request with REQUIRED chart notes and lab results. If prior authorizations are required by your insurance provider, those must be in place prior to scheduling. We may require more information than is provided in the initial referral (prior to your first visit) and we will work with you and your referring office and/or other offices to get that information prior to your visit.

INSURANCE: We are in-network for all commercial insurance plans in our market including: Providence, Healthnet, PacificSource, Atrio, Aetna, Regence Blue Cross Blue Shield, CIGNA, First Choice Health, MODA, Pacific Source, Samaritan Choice, United Health Care and all others in this geographical area. If you are a City of Salem or Oregon State employee we will be in network with your plan. If your plan is not listed here, please feel free to call our Business Office Specialist at 503.589.0565 ext. 115 to confirm acceptance of your plan. Please call Monday thru Thursday between 8am and 4pm with insurance questions.

PATIENT RESPONSIBILITY: It is always the patients responsibility to confirm specific coverage with their insurance plan as insurance companies offer many plans and the coverage limits are specific to each plan type. Please call your insurance company customer representative to find out if a specific visit type or procedure is covered and what your copay/co-insurance payment will be. Please also be aware of your insurance plan annual deductible and if that threshold has been met (prior to each visit) so that we can make arrangement for payment for your visit at your visit when necessary. We do require co-pays when applicable at each visit. Our Business Office Specialist can assist with determining what your visit type will be if you have any questions.

Patients are seen by appointment only. 

Existing Patients

Two way communication between clinical staff and patients is achieved by signing up for our secure patient portal. Lab result documents can be sent to you upon request, and non-emergency questions can be sent to the clinical team. We respectfully request that you call the office with any scheduling questions or concerns as we are unable to manage schedule changes thru the patient portal. Any appointment cancellations must be confirmed a minimum of 24 hours prior to your scheduled appointment so that other patients may take available spots.
